available to medical professionals and students. Fill in one of the two
forms below, sign and date it, and have your sponsor sign and date it.
Then return the form to the Shock Society Membership Committee as
indicated.
Print either version and type or
carefully handwrite your application. Do not forget to sign and date the
application form. Follow the instructions to mail the form to the
Membership Committee. |
